Community Health Assessment Advisory Group. These key issues were identified through the findings from four assessments conducted between July 2010 and July 2011. This process was based on the National Association of County and City Health Officials’ (NACCHO) Mobilizing for Action through Planning and Partnerships (MAPP) model.
