From what I've seen, it sounds like the surface doesn't actually count as the surface in getfixedboi, IIRC, but I could buy the forest pylon in the grassy area near hell instead.

You dont, like the game doesnt give you any good item against the spread except for bombs and powders, best thing to do is to Isolate the biomes. Dig it all up. At that rate, you're better off ignoring it. There's a reason they don't give you the clentaminator, the surface is not meant to be conquered.
